In this declaration, I hereby agree not to hold the Singapore Table Tennis Association (STTA) or their appointed staff and officials responsible or liable for any mishaps, injuries, loss of life, or damage to property arising from or in connection with the aforementioned activities. I also agree to indemnify the aforementioned organizers, their staff, and officials from any actions, claims, damages, costs, and expenses brought by any person in connection with these activities.

By participating in the activities, events, programs, tournaments, and/or using the training and sports halls organized by the Singapore Table Tennis Association (STTA), I grant STTA and its affiliates irrevocable and unrestricted permission to film, photograph, and/or video record my appearance and voice during such activities. I also grant STTA the right to use my image, likeness, and voice without further notification or compensation, in any manner they deem fit. This includes use in promotional materials, publications, websites, social media, advertisements, and any other media, whether currently known or developed in the future.

I agree not to make any claim, sue, or hold the STTA, its affiliates, employees, representatives, or volunteers responsible for any claims, liabilities, damages, costs, or expenses, including attorney's fees, arising from the use of my image, likeness, or voice, or from any claims of libel, defamation, or invasion of privacy related to such usage.

By participating in STTA's activities, events, programs, tournaments, and/or using the training and sports halls, I acknowledge that I have read and understood this consent statement and voluntarily agree to its terms.
